What does the membership include?

Your membership covers all primary care services — preventive visits, sick visits, chronic disease management, genetic consultations, care coordination, and provides you with direct physician access via phone, text, and secure e-mail. See the Services page of our contract for the full list.

Do I still need health insurance?

Yes. Your DPC membership covers primary care only. We recommend a high-deductible health plan (HDHP) or health-sharing plan for specialist visits, hospitalizations, and emergency care. Many patients save significantly by pairing DPC with a lower-premium HDHP. To use HMO insurance effectively, you will need to maintain a relationship with a traditional "in-network" PCP.

How quickly can I be seen?

Same-day or next-day appointments are standard. For urgent matters, you can reach your physician directly by phone or text — no hold music, no triage nurse.

What is the genetics component?

As part of your care, we offer genetic testing and analysis to identify inherited health risks, optimal medication selections (pharmacogenomics), and personalized prevention opportunities. Genetic testing is not included in your membership fee but should be considered an essential add-on.

Can I transfer my existing records?

This is typically not necessary because we will build your new chart "from-scratch". So we have a complete picture of your health history, we will request specific records from your prior and currently active Medical Providers (as needed) and review them before your first visit.

Is there a contract or cancellation fee?

No. You are not required to sign any long-term contracts. Membership is month-to-month and can be cancelled with 30-days notice. *If we are not meeting your expectations, you should cancel your membership.

How to get started

Schedule a Meet & Greet

Schedule a free 15-minute office visit with one of our physicians to make sure we're the right fit for you. No commitment required.

Complete your intake

Click the downloadable "Office Forms" link and print the "Medical History", "Medications and Allergies" and the "HIPAA Notice". Carefully fill them out and bring them to the meet-and-greet visit so we can best determine how we might meet your medical care needs.

Choose your membership

Select the plan that best fits your needs — Individual, Individual and Spouse, or Family. We'll walk you through the options and discuss available discounts. If you select us as your "Concierge PCPs", you will be given a Patient Enrollment form to complete.

Your First "Real" visit

At your first "real" visit, we will carefully review your Medical History, perform a comprehensive physical exam and the listed testing. We will also walk you through the process for ordering WGS genetic testing. Expect 60 minutes or more for this visit.

Enlightened Primary Care is a Direct Primary Care (DPC) Medical Practice. Your monthly membership covers all primary care services directly — no copays, no deductibles, no surprise bills. We recommend pairing your DPC membership with a high-deductible health plan or health-sharing plan for Specialist and Hospital (major medical) coverage. The annual cost savings from changing to a high-deducable Insurance plan may off-set our service fees completely. *You may be able to get enhanced "Executive-level" Concierge Medical Care at no extra cost.
